We seek your kind attention to check with nearest branches / KLHQ Customer Service on your Distributor membership name, spouse’s name (if any) and contact number in order for the company to process the Certificate & Trophy plate printing.

Please complete FORM 165 should there be any changes latest by 30th December 2011. Amendment after this deadline shall deem not effective for this Award Presentation.
